Please note that birth cards must be purchased along with a birth certificate.  You cannot purchase a birth card as a single purchase.

 

Birth Certificate/Birth Card and Death Certificate

Birth Certificates and Birth Cards - Individuals born in Louisiana may obtain a certified Birth Certificate from the Clerk of Court's office. 

Individuals born in Louisiana may also obtain a certified Birth Card from the Clerk of Court's office. This 3" x 4" document has the person's name, date and parish of birth, sex, mother's maiden name and father's initials listed as well as the Vital Records birth number.

Death Certificates

Certificates for deaths registered in Louisiana from July 9, 2012 to present may be obtained from the Clerk of Court's office. Death certificates are usually available within 90 days of the death.

An Application for a Certified Copy of Birth/Death Certificate Form #PHS520F  must be filled out and signed by the applicant. This form is available at the Clerk of Court's office.

Identification Requirements

Applicant must present one form of identification as described below. All identification presented to obtain a birth record must be current.

A current state issued Driver's License that includes a photograph which clearly identifies the applicant as the same; OR

A current state issued identification card that includes a photograph which clearly identifies the applicant as the same; OR

A current U.S. passport with photograph which clearly identifies applicant as the same; OR

A current U.S. military photo identification card which clearly identifies applicant as the same; OR

Students (high school or below), a current school yearbook or a current official school

Identification document with a photograph that clearly identifies applicant as the same.

As per Louisiana Vital Records, all identification must have dates of issue and expiration clearly visible on the ID. 

Identification with dates missing, unclear, or names or photos worn away will not be accepted.

Identification must be legible and may not be torn, broken, or mutilated.

Eligibility

Any immediate family member (mother, father, current legal spouse, child, grandparent or sibling) may obtain a registrant's birth certificate, death certificate or birth card by filling out and signing an Application For A Certified Copy Of Birth/Death Certificate Form #PHS520F. The relative must present their own required current photo identification.

An individual that has a certified document of custody regarding a minor from a Court proceeding may obtain a birth record for the minor registrant named in the document.

An individual named as a beneficiary on a deceased person's insurance policy is also eligible to apply for a copy of the deceased person's death certificate. Applicant must present an original of the insurance policy which clearly names the deceased person as well as the beneficiary.
